A RESOLUTION APPROVING AND CONCURRING THE AUTHORITY GRANTED TO


MR. DENVER CUSTODIO, SK KAGAWAD OF BARANGAY 93, ZONE 8 DISTRICT 1 OF
MANILA, TO ACT ON BEHALF OF OUR BARANGAY IN APPLYING FOR
“PLDT DSL UNLI PLAN 1299” FOR OUR ENTIRE PERIOD OF ADMINISTRATION.

WHEREAS, the appointment of the above mentioned positions are necessary for the
DEVELOPMENT OF OUR BARANGAY;

WHEREAS, the appointment is necessary to lessen the expenses of our barangay in


applying for PLDT as an individual applicant rather than as a business applicant;

WHEREAS, the persons named above are qualified on said petition in accordance to
the relevant provisions of the 1987 Philippine Constitution and any existing statutes
pertinent to this matter to represent our barangay with the above-captioned
authority because the above-named individual is an ELECTED SK KAGAWAD;

WHEREAS, the above-named is given the authority to apply, transact, execute, sign
and do any or all acts incidental to such application;

WHEREAS, the above-named is allowed to apply only for PLDT DSL UNLI 1299 in
which our barangay will use his name to transact with PLDT;

WHEREAS, the above-named consented with this arrangement to use his name in
applying with PLDT on behalf of our barangay but will not be personally liable to
any and all debts that our barangay will be incurring in the future;

WHEREAS, the said PLDT DSL UNLI PLAN 1299 will be automatically requested to
cut its connection with PLDT upon end of the present administration’s term of office
at the instance of the above-named individual or by the Barangay Treasurer,
Ms. Jasmin Mayo;

NOW THEREFORE, on motion of all bodies, the above named individuals are NOW
our duly authorized representative to appy with PLDT.

RESOLVED, as it is hereby resolved to solemnly and unanimously approved with the


concurrence of at least 2/3 votes of all the Sangguniang members, including the SK
Chairman;

RESOLVED FURTHER, to furnish copies of this resolution to all Sanggunian


Barangay (“SB” for brevity) Councilors and Members of the Sangguniang Kabataan
(“SK” for brevity).

RESOLVED FURTHERMORE, that copies of this resolution be sent to the Office of


the DILG and other concerned agencies for their information and advice.
