2nd PTA MEETING AND 1ST GRADING CARD DISTRIBUTION
Date: October 1, 2019
Time: 10:00 A.M
Venue: CAYPOMBO GYMNASIUM AND DESIGNATED ROOM
Agenda:
1. AWARDING OF PERFECT ATTENDANCE AND ACHIEVERS
2. PTA COLLECTION (Legal Fees)
3. School’s Rules and Regulations
4. Form 137 and NSO Birth Certificate
5. ATTENDANCE
6. DISTRIBUTION OF PROGRESS REPORT
7. ELECTION OF PTA OFFICERS PER CLASS
Proceedings:
1. Meeting called to order at 10:00 a.m. at the designated room.
2. Awarding of perfect attendance and achievers.
3. Reiterated the following school’s rules and regulations:
 Attendance of the learner.
 Students who fail to wear proper uniform/attire will write in the student log
book.
 Students’ tardiness and absences will be recorded by the class monitor so that
in case the parents would want to know the status of their children, there is a
record to serve as basis.
 Bringing of cellphone is not allowed
4. Reminding the parents to follow up the form 137 of their children from
their previous school and those parents whose children have not submitted their NSO birth
certificate.
5. Informing the parents about the PTA fees and School miscellaneous fees.
6. Distribution of Report Cards.
7. Election of PTA Officers.
