I, JUAN DELA CRUZ, Filipino, of legal age, single and a resident of


_____________________________, after having been duly sworn to in
accordance with law do hereby depose and say:

1.- THAT I am the complainant in Criminal Case No. 123456789 for


Violation of Art. 293 (Robbery Extortion) against PEDRO SAN JUAN which case is
now pending trial in the Regional Trial Court, Branch __, ________;

2.- THAT I have completely lost any interest in further prosecuting said
case against PEDRO SAN JUAN;

3.- THAT I am executing this Affidavit to attest the truth of the foregoing
statements and to support my move for the outright dismissal of the case against
PEDRO SAN JUAN;

4.- THAT no one has ever forced, threatened or promised me of any


consideration in executing my affidavit and the same is my free act and deed;

5.- THAT all the foregoing statements have been read and explained to
me and I have fully understood all the contents of my Affidavit.
