INSTRUCTIONS
(Read carefully before filling the form)
1. Examination admission form duly complete in all respects together with fee receipt should reach the
Controller of Examinations, Islamia College Peshawar on or before the last date with Normal fee notified
for the purpose failing which late/double fee will be charged.
2. Examination fee should be deposited in the nearest designated branches of United Bank Limited (UBL)
Account Number: 231098405
Account for Islamia College Peshawar. Fee remitted through Bank Draft in favor of the
Treasurer, Islamia College Peshawar must mention name of the candidate, father’s name and
examination on its back side. Examination fee by money order/cheque shall not be entertained.
3. Each candidate (Male/Female) must staple/paste three (03) identical passport size photographs,
duly attested, at the specified places of the admission form (Name and father’s Name must be mentioned on
the back side of photographs).
4. Every candidate must keep his/her Computerized National Identity Card with him/her in the Examination
Hall while taking the Examination along with his/her Roll Number Slip issued
By the University.
5. Examination center will be allotted by the University where feasible and not on the choice of the
c a ndida t e . Cares of addresses are not entertainable in cases of change of center
permissible under the rules.
6. The candidate, who has passed his/her graduation from some other University, should submit the
Original Migration certificate along with other documents.
7. Any incorrect statement made in this form may lead to disciplinary action or cancellation of
admission at any time.
8. Possession of Mobile phone is not allowed in examination center. If anyone found in possession,
his/her examination will be cancelled besides forfeiture of Mobile Set.
