AFFIDAVIT OF LOSS

I, SHERILYN ANN INFANTADO, of legal age, single, Filipino citizen, and a resident of
Daisy Road, Guiwan, Zamboanga City Philippines, after having been duly sworn to in accordance
with law, hereby depose and state:

That I the surviving daughter of the late Ma. Elena Marilyn M. Infantado who died on May
11, 2020 in Zamboanga City;

That after her death, we were able to find a copy of the Deed of Absolute Sale executed
by and between Spouses Juan Gonzales and Consolacion G. Gonzales in favour of our late
mother, Ma. Elena Marilyn M. Infantado on October 20, 2006. Accompanying the said Deed of
Absolute Sale is a copy of the ONETT CONFIRMATION LETTER dated March 1, 2010 addressed to
Mr. Juan Gonzales;

That in the said ONETT Confirmation Letter, it states the OR Nos. 03344510 and 03344511
as proof of payment for Capital Gains Tax (CGT) and Documentary Stamps, respectively;

However, in finding the said documents, nowhere in the folders and envelopes left at
home, where these documents were found and all other personal documents were safely kept,
the original and duplicate copy of the manually issued or expired CAR and the Official Receipts
bearing nos. 03344510 and 03344511 were found. I even tried to check her office table with the
hopes that the said documents were just safely kept, however my efforts were in vain;

Hence, I now consider it lost and beyond point of recovery however to no avail; and

That I am executing this affidavit to attest to the veracity of the foregoing facts and for all
legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand August 4, 2020, in the City of
Zamboanga, Philippines.
