AFFIDAVIT

I, LORENZO B. GARCIA, of legal age, Filipino citizen, and a residence


address at 26 Greenwood St. Villa Luisa Subd. Sta. Lucia, Sta. Ana, after having
been duly sworn to in accordance with law, depose and state:

1. That I am the driver/owner of a motor vehicle which is more


particularly described as follows;

Make and Type : Toyota Fortuner 4x2 2.0


Chassis Number : MHFJB8GS6G1514223
Engine Number : 2GDC094391
Plate Number : CDJ8477
Year : 2017

That on March 2, 2020, at 5:00 o’clock in the afternoon, while the said
vehicle was parked at the SM Pampanga, and when I came back to the said
vehicle I was surprised when I see the vehicle the passenger side panels, driver
side panels, front bumper, back bumper, was scratches and vented; the hood
was dented; and the rear side panel of the trunk was damage;

That the damage incurred on the said car was not caused by a vehicular
accident.

That I am executing this affidavit to attest to the truth of all foregoing and
for whatever legal purpose it may serve.

Further affiant sayeth none.
