AFFIDAVIT OF LOSS

, after having duly sworn to in accordance with
law, hereby depose and say that:
1. I am the buyer of Isuzu Cargo Truck with Plate No. in
the Deed of Absolute Sale dated , executed by and between me
and the registered owner of the subject vehicle, and seller,

2. On , during our execution of Deed of Absolute Sale,
told me that he lost the Original Certificate of Registration
for the said Isuzu Cargo Truck No. issued by
, and what was given to me was only a photocopy of the said
Certificate of Registration;
3. I even assisted him in looking for the said original certificate of
registration, but despite our diligent efforts exerted to locate and/or recover the
said document, it can no longer be found and thus, considered it lost for all
legal purposes;
4. As such, I am executing this Affidavit of Loss to attest to the truth
of the foregoing, and to support my application for the replacement and/or
renewal of the said Certificate of Registration.
