AFFIDAVIT OF LOSS

I, ________________________ , Filipino, of legal age, married, and a resident of


___________________________, after having been duly sworn to in accordance with law,
hereby depose and say that:

1. I am the authorized representative of _______________________, the true


and registered owner of a ____ square meter parcel of land situated in
____________________ and covered by Transfer Certificate Title No_______________
2. As such authorized representative, I was the one entrusted to keep the
said original copy of the Transfer Certificate of Title No. ____________;

3. Sometime in ____________, I lost/misplaced the said original title inside in


our house, and despite diligent efforts exerted to locate the same, I could not
remember exactly where I placed/left it, thus, considered it lost for all legal purposes;

4. The said TCT has not been given or surrendered to anyone as collateral
for any loan;

5. As such, I am executing this Affidavit of Loss to attest to the truth of the


foregoing and for whatever legal purposes this may serve.
