AFFIDAVIT OF LOSS

I, MERJUNE N. AMOLAR, single, Filipino citizen, of legal age and


presently residing at Brgy. Aquino Nobleza East, Janiuay, Iloilo after
having been duly sworn to in accordance with law, hereby depose and
state:

1. That I am holder of a CASH CARD issued under my name issued


by ORIENT lending corp.;

2. That the last time I used such Card was on September 30, 2020
in Iloilo City;

3. That after, I discovered that such card was lost and can no
longer be found despite diligent efforts to locate the same;

4. I execute this affidavit to attest the truth of the foregoing and


to support my request for the replacement of such cash card.
