AFFIDAVIT OF LOSS

I, RYAN AGUILAR, of legal age, single, Filipino citizen and a resident of


Kabasalan, Zamboanga Sibugay, Philippines, after having been duly sworn to in
accordance with law, hereby depose and say that:

1. I am a beneficiary of 4ps;

2. As such, I was issued a corresponding 4ps ATM Cash Card;

3. Said ATM Cash Card was usually kept inside my wallet for safekeeping and easy
access;

4. However, due to inadvertence and/or oversight, I discovered that the above-


mentioned ATM Cash Card was no longer inside my wallet and its whereabouts is
nowhere to be found.

5. I exerted diligent efforts to locate for the same in other places, but all such
endeavors proved futile and to no avail;

6. For all intents and purposes, I am now declaring the above-mentioned Card lost
and beyond recovery;

7. I execute this Affidavit to attest to the truth of the foregoing facts based on my
personal knowledge, for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 23 rd day of January


2023 at Ipil, Zamboanga Sibugay, Philippines.
