Antonio, Brgy. Zone 12-A Talisay City, Negros Occidental, proprietor of DVS Construction Supplies &
Services, do hereby name, constitute and appoint HENRY BAÑARES of legal age, Filipino and a resident
of Brgy. Zone 14 Talisay City Negros Occidental to be my true and lawful attorney in-fact to act and in my
stead, to do and perform the following acts, to wit:
To represent on my behalf for a conciliation conference at National Labor Relations Commission Office
(NLRC) on Tuesday, March 3, 2020 at 2:00 pm.
HEREBY GIVING AND GRANTING unto my said attorneys-in-fact full power and authority to
represent, as fully to all intents and purposes as I might or could lawfully do if personally present and
hereby ratifying and conforming all that my said attorney-in-fact shall lawfully do or cause to be done by
virtue of these presents.
DOCUMENT SHALL be construed and interpreted as a limited special power of attorney which is
valid only for the purposes stated herein and may not be used for any other transaction or by any
person not named.
THIS SPECIAL POWER OF ATTORNEY shall expire if revoked by the undersigned in writing.
FURTHER, I SAMUEL SIOTE, hereby revoke all Special Powers of Attorney that I may have
executed before the date of this Deed.
