AFFIDAVIT

I, VICTOR A. FERROLINO JR., of legal age, single, Filipino and resident of Brgy.
Bongbong, San Francisco, Southern Leyte, after having been duly sworn to an oath in
accordance with law, hereby depose and say:

1. That I am applying for the position of Clerk of Court II at San Francisco Municipal
Trial Court, San Francisco, Southern Leyte;

2. That one of the requirements of the said application is a latest Medical Certificate
and Laboratory Results. However pending for the submission of the above
requirements, I am declaring that I am physically fit to work for this position and
willing to submit myself for such medical examination, if appointment permits;

3. That I executed this affidavit freely and voluntarily in order to attest the truth
and veracity of the foregoing, for the purpose of my application for employment
and for whatever legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set our hands this 7 th day of October 2017 in the
Municipality of Sogod, Southern Leyte, Philippines.
