Dear Mr. Ombay:

It has come to the attention of the management that SINCE September 3, 2016, through
RELIEVE ORDER dated September 2, 2016, you failed to comply and report at DAVAO SECURITY
AND INVESTIGATION AGENCY, INC. Head Office for your possible reassignment.

You have been remiss in your duties and responsibilities by failing to report without
informing your immediate superiors of your whereabouts and as to why you did not comply
with the said RELIEVE ORDER.

Failure to follow this Order shall tantamount to a waiver of your right to be heard and
present evidence or witness/es. For strict compliance.
