SUBJECT : Letter of Apology

Dear Sir,

This is to extend my sincere apology on the incident occurred last December 19, 2019
(Pulling out of our copper pipe scrappage) and I take full responsibility on this matter. I
instructed my project foreman to disposed those scrap intended for food allowance of my work
force. I informed Engr. Mark and asked for him to sign the pull-out form and be logged with the
guard on duty. I was not aware that this should be checked by the warehouseman. Again, my
apologies on my negligence although it was an honest to goodness mistake.

Rest assured that this won’t happen again.
