to your education agent or EQInternational@qed.qld.gov.au. Please note, incomplete applications cannot be assessed. EQI
recommends you read the Standard terms and conditions (https://eqi.com.au/apply-now/terms-and-conditions) and the EQI
policy and procedures (https://eqi.com.au/for-students/policies-procedures) available on our website, before you submit your
application.
You should submit your application at least 3 months prior to your proposed commencement with EQI, please refer to the
EQI application cut-off dates (https://eqi.com.au/apply-now). Visa processing times vary for each country. For further information you
should contact the Department of Home Affairs and the relevant Australian visa processing office, for your country.
Students applying to Queensland Academies (Appendix C) must submit their application to EQI at least 1 week prior to the
Queensland Academies application closing date. Visit https://qa.eq.edu.au/apply/ for application closing dates.

Privacy:
Your personal information is collected on this form to assess your application for enrolment into a Queensland State school. EQI may contact your
previous education provider to verify information provided in this form. If your application is successful, the information you provide will be used to
select an appropriate school for you, to process your enrolment in a Queensland State school, facilitate your ongoing enrolment and for the purposes
of the international student program, generally, including discharging EQI’s duty of care to students. If you request EQI to arrange Overseas Student
Health Cover (OSHC) for you, your personal information will be used, disclosed and exchanged with the OSHC provider for that purpose. If you request
EQI to arrange homestay accommodation for you, your personal information (including ‘Medical’ and ‘Behaviour’) will be used and disclosed for the
purposes of selecting and arranging an appropriate homestay provider and there will be an ongoing exchange of information between homestay
providers and the school/EQI, including through secure online services such as QParents. Some of the information requested on this form will be used
and disclosed by EQI to comply with our obligations under Australian laws (such as Education Services for Overseas Students Act 2000 (‘ESOS Act’) and
National Code of Practice for Providers of Education and Training to Overseas Students 2018 (‘National Code’)). EQI may disclose information about you
to Commonwealth Government agencies including the department responsible for the Migration Act 1958 and the department responsible for the ESOS
Act. EQI may also record, use and disclose your personal information to third parties where authorised or required by law.
Students who are resident in the EU are required to complete a GDPR personal data collection and consent disclaimer which must accompany this
application form. Your information will be stored securely. If you wish to access or correct any of the personal information on this form or discuss how
it has been dealt with, please contact EQI in the first instance. If you have a concern or complaint about the way your personal information has been
collected, used, stored or disclosed, please also contact EQI in the first instance.

5. Visa And Overseas Student Health Cover
Students are required to obtain a valid Australian student visa for the duration of their EQI program.#

8. Medical
If your application is successful, the medical information you provide will be given to your school and your homestay family (if you will
not be living with your parent or approved relative), so that any medical conditions can be appropriately managed at school (including
school excursions, school camps, sports and other school activities), and at home (if you are in a homestay), and in the event of
an emergency, this information can be promptly provided to the medical practitioners treating you. This information will be treated
confidentially, where possible, but EQI’s main priority is your health and safety. It is essential that you provide all relevant information
to EQI. If your application is successful and you are enrolled in a school, you will be required to inform the school as soon as you are
aware of any new medical conditions or a change to medical conditions. Should you need to take medication during school hours, an
Individual Health Plan, including Emergency Health Plan if relevant, or an Authority to Administer Medication Form must be completed
each year and retained at the school office.

APPENDIX C – Queensland Academies
C1. Course selection

The Queensland Academies have a single point of entry into the Academy program, which is the start of year 10. There is no mid-course
entry, unless covered by the conditions outlined at: https://qa.eq.edu.au/apply/year-10-admissions-policy/.

C3. Queensland Academy testing arrangements


Entry to the Queensland Academies is via a selective entry process.
The Queensland Academies Admissions Office will invite eligible EQI applicants to submit a Queensland Academies application and
also register for an Entrance Test designed by Edutest. Application fees apply.
Applicants can sit the Entrance Test at their selected Queensland Academies Campus. Alternatively, applicants can arrange to sit the
Entrance Test remotely for an additional remote testing fee.
Based on the academic data provided and Entrance Test results, applicants may be short listed for an interview. Applicants are
required to submit a sample of past work and Statement of Academic Journey at the student interview.
