AFFIDAVIT OF UNDERTAKING

I, NIMFA P. TIEMPO, of legal age, Filipino and a resident of Purok


Galingan, Mankilam, Tagum City, Davao del Norte, after having duly sworn
to in accordance with law hereby depose and state that:
1. I am processing the Business Permit of my Dormitory located at Purok
Luzvimin, Magugpo South, Tagum City;

2. Part of the requirements thereof is the Fire Certification issued by the


Bureau of Fire Protection;

3. I undertake to construct a fire exit stair within the days as may


granted to me by the Bureau of Fire Protection;

4. I also undertake to provide firehouse box with firehose;

5. I am executing this affidavit to attest to the truth of the foregoing


statements and for the issuance of the Fire Certification from the
Bureau of Fire Protection.

IN WITNESS WHEREOF, I have hereunto affixed my signature this
