RE: Ms. Patricia Styles, 63 years old
I am referring to your care Ms. Styles, who is suspected to have complications related to
pericarditis. She requires an urgent assessment and management for the said condition.
Today, during home visit, Ms. Styles is experiencing shortness of breath, chest pain, and easy
fatiguability. She was admitted in Green Valley Hospital with the same complaints a week ago
and has been diagnosed with Type B influenza and pericarditis as revealed by her
echocardiogram and blood workups. Now, there is a high probability to have relapse of
pericarditis.
Ms. Styles has had Diabetes Mellitus type II since 2009 for which she takes oral hypoglycemic
drugs such as Metformin and Glipizide. She has been a hypertensive for 9 years and was
prescribed Carpinol to control her blood pressure. It is also worth noting that she is suffering
from depression for the last 6 years.
It would be greatly appreciated if you could assess Ms Style’s condition immediately and plan a
treatment regime to resolve her pericarditis. Please note that she attends counseling for her
mood swings and diabetes mellitus management.
