AFFIDAVIT OF LOSS OF PASSPORT


I, RAMON KHALIL C. ERUM IV, Filipino citizen, of legal age and presently
residing at no.99 Isla Suerte, Bucana, Davao City, after having been duly
sworn to in accordance with law, hereby depose and state:
That I am a holder of Philippine Passport No. P00000058 issued on
January 4, 2018. That sometime around October 12, 2019, I noted
that the said passport was missing.

That the circumstances surrounding the loss of my passport follows:

On October 12, 2019 in the airport of Johannesburg, South Africa I


presented my passport to the customs officer before boarding my
flight to Paris, France. Upon arriving to Paris, France at the Customs
Section of the airport, I was shocked to have discovered that my
passport was missing from my bag. I contacted the Customs Officers
in Johannesburg on the whereabouts of my passport to no avail.
Also, I have reported the loss of the said passport to the authorities
here in Paris, France.

That I execute this affidavit for the purpose of giving true and correct
statement surrounding the loss of my passport;

That further affiant sayeth naught.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 14 th


day of October 2019 at the Philippine Embassy in Paris, France
