They give their full consent to Victoria De Gulan to process the exhumation of their parents' remains from the specified lot. They also consent to Eternal Gardens Memorial Park Corporation to exhume and rebury the remains in the same lot. Additionally, they hold Eternal Gardens Memorial Park Corporation harmless from any liabilities arising from the exhumation.

RE: ONE AND THE SAME PERSON

all Filipinos, of legal ages, having been duly sworn in accordance with law, hereby
depose and state:

1 We are the living children of late , MERCEDES RAMOS &


EDILBERTO RAMOS who died on ________________ and ________________,
respectively, and were buried at Eternal Gardens Memorial Park Corporation;

2 We are willing and giving our full consent to Ma. Victoria R. De Gulan
to process the exhumation of our mother and father;

3 We are also willing and giving our full consent to Eternal Gardens
Memorial Park Corporation to exhume the remains of the late MERCEDES
RAMOS & EDILBERTO RAMOS buried in the lot specifically described as
“TRANSFIGURATION (FE) SECTION: B BLOCK: 001 LOT 001” and rebury at the
same lot;
 
4 We hold Eternal Gardens Memorial Park Corporation free and
harmless from any and all liabilities, claims, damages, suits or cause of action
and other expenses arising out of or in connection with the exhumation.
