SWORN CERTIFICATION OF RESIDENCY


I, MA. ADELAIDA S. RESTAURO, of legal age, married, and a resident of San Isidro,
Talisay City, Cebu, Philippines, after having been duly sworn in accordance with law,
hereby depose and say that:

1) I am the duly elected Punong Barangay of Barangay San Isidro, City of Talisay,
Province of Cebu, Philippines.

2) As a Punong Barangay, I am authorized to certify who are bona fide residents of


our barangay.

3) As such, I acknowledge and certify that IAN AUMENTO CUIZON, of legal age,
married, is a resident of Zone 3, Largo Compound, San Isidro, Talisay City, Cebu
for at least six (6) months prior to the issuance of this certification.

4) I further certify that IAN AUMENTO CUIZON has been residing in the barangay
for 24 years already.

5) Although he is not a registered voter in the barangay, it is understandable


because of the nature of his work as a seafarer. His work schedule is
unpredictable, thus, preventing him to maintain his registration as a voter in the
barangay.

6) To further prove his residency in the barangay, attached herewith and marked
as Annex “A” hereof is the house location sketch of the residence of IAN
AUMENTO CUIZON.

7) As such, I am executing this affidavit to attest to the truth of the foregoing facts
and to establish his residency as support to his petition for annulment.
