AFFIDAVIT OF LOSS

I, XXX, of legal age, married, Filipino and a resident CCCC City, after being duly
sworn in accordance with law, hereby depose and state: That-

1. I am the Corporate Secretary of YYY., a corporation duly organized under the


laws of the Republic of the Philippines, with office address at DDD City

2. Given the above-mentioned position, I knew that the Company hired a third-
party Auditor to conduct the financial audit and prepare an Audited Financial
Statements to be submitted to the relevant government agencies.

3. In 2018, we filed with the Office of the Bureau of Internal Revenue our
Audited Financial Statements for Y2018.

4. The Head of our Accounting Department resigned from his position in


______ and upon verification of our accounting records/documents, we have
not found the 2018 Audited Financial Statements.

5. We exerted reasonable efforts to locate said file/records and even contacted


our former Accounting Head to inquire as to the whereabouts of said records,
but our efforts found futile.

6. I am executing this Affidavit to attest to the truth of the foregoing and for the
purpose of securing a certified true copy of said YYY’ Audited Financial
Statements for Y2018 with the Bureau of Internal Revenue and other relevant
government agencies where we submitted the same.
