AFFIDAVIT OF LOSS

been duly sworn to in accordance with law, depose and state that:

1. I am a student of TSKI SKILLS AND KNOWLEDGE INSTITUTE, INC.;

2. As such, Technical Education and Skills Development Authority


(TESDA) issued in my favor a Competency Assessment Result
Summary for NCII in Microfinance Technology;

3. After recent inventory, said Competency Assessment Result


Summary for NCII in Microfinance Technology was misplaced
and got lost at the office of TSKI SKILLS AND KNOWLEDGE
INSTITUTE, INC.;

4. Earnest efforts were exerted to locate said document, but despite


of diligent search, it cannot be found and the same is now beyond
recovery;

5. I am declaring the aforementioned document loss for all legal


purposes;

6. I am executing this Affidavit of Loss to attest to the truth of the


foregoing and to support my application for the re-issuance of the
said Competency Assessment Result Summary for NCII in
Microfinance Technology by the Technical Education and Skills
Development Authority (TESDA).
