AFFIDAVIT OF DISCREPANCY

I, NERISSA A. ARABEJO, of legal age, Filipino, married and a resident of Villa Verde,
San Miguel, Tagum City, Davao del Norte, Philippines, after having been duly sworn to in
accordance with law, hereby depose and say:

1. That I am the same person whose person whose personal circumstances are above-
mentioned and who caused the preparation of this affidavit;
2. That upon verification of my Marriage Contract, my year of birth was erroneously entered
as 1971 instead of 1969 which is the correct one;
3. That the said entries should be corrected by way of this affidavit in order to avoid confusion
and to prevent further erroneous entries in subsequent papers and documents;
4. That I hereby attest to the truth of the foregoing statements herein made without mental
reservation whatsoever in order to inform the proper authorities of whatever legal purpose
this affidavit may serve best;
IN WITNESS WHEREOF, I hereby affix my signature this 23 rd day of December, 2010 at
Tagum City, Davao del Norte, Philippines.
