Dear Sir/Maam:

Greetings of Peace and Prosperity!

I am James R. Potente, who just passed the Social Worker's Licensure Examination last February
10-12, 2021. I am writing this letter to your respective office to ask for permission to allow a
friend of mine to claim my certificate of board rating, and other certifications on my behalf.

The reason for me not to come on the day of my scheduled appointment is that its hard for me
to come, because im also currently in Marawi City now, and is newly employed in a private
school.

(Rest Assured that i will be making an authorization letter including the important documents
for verification and identification purposes, lile my NOA, valid ID, screenshot of our oath
peinted.)

I hope and expect for your good Heart and kind consideration.
