Dear Sir/Madam:
This is to authorize ______________ to obtain my Clearance from your office, as a requirement for the
application of a Notarial Commission. I reside in _________________ and could not readily travel to
Manila to obtain the same requirement. I made a call today at your good office inquiring if it is
possible to authorize a representative instead to obtain the clearance on my behalf, and said that it was
possible. Thank you for your kind consideration.
