Dear Atty. Nacional,

I am writing to formally request for an Office of the Bar Confidant


Clearance (OBC Clearance, for brevity) for the renewal of my
Notarial Commission.

I am Atty. Shain Ann C. Castillo who was admitted to the Philippine


Bar on 2017 with Roll of Attorney Number 71234. My Notarial
Commission will expire this December 2020, thus prompting me to
file for its renewal.

In this regard, I am requesting for an OBC Clearance which is a


requirement for the renewal of the aforesaid Notarial Commission.
