1. I am the Moderator of ​ Musmos Ateneo for School Year 2020-2021 . 
 
2. In  accordance  with  the  Anti-Hazing  Act  of  2018  (Republic  Act  No.  11053),  I  take  full 
responsibility for monitoring the activities of ​Musmos Ateneo 
 
3. I  am  aware  that  Sec.  7  of  the  Act  provides  that  “In  case  of  violation  of  any  of  the 
provisions  of  this  Act,  it  is  presumed  that  the  faculty  adviser  has  knowledge  and 
consented to the commission of any of the unlawful acts stated therein."  
 
4. I  understand  that  on  the  basis of Sec. 7 of the Act I may be held jointly and solidarily 
liable  with  the  responsible  officers  and  members  of  ​Musmos  Ateneo  for  violations 
of  the  Act,  unless  I  am  able  to  present  evidence  to prove that I did not know and/or 
did not consent to the unlawful act. 
 
5. In  case  I  am  found  liable,  I  will  indemnify  Ateneo  de  Manila  University  and/or  its 
employees  for  any  and  all  costs  or  liabilities,  including  but not limited to damage or 
injury  to  persons  or property, that may result from or may be incurred in the course 
of  the  conduct  of  the  activities  of    Musmos  Ateneo   found  to  be  in  violation  of  the 
Act.
