III. PROBLEM PRESENTED
The client is experiencing an on and off fever and a cough. He appears to be thin, pale and
weak. He has nobody to asked for some help to buy some medicine and for his food.
IV. BACKGROUND INFORMATION
A. The Client
The client is 16 years old appeared to be thin, pale and weak. He always feels
uncomfortable and irritated because of his sickness. He feels so helpless and hopeless
because he has nobody to ask for help. The client is hardworking person he works as a
helper in a lumber yard and used to work as a sikad driver in the afternoon. He do sideline
job because his salary is not enough to sustain his daily expenses. He is the only one
providing for his needs.
The clients wants to be treated on his illness but he has no one to ask for help. He
want to become well so that he can work and earn money to sustain his daily expenses. The
client is the only one sustaining for his daily expenses. Despite of the client situation he still
force himself to work just to earn money that he used in buying food.
B. The Family
The client’s father has no work and his mother was already dead. His father could
not work because he was weak and very thin and suffering from tuberculosis.His father was
detected of having a tuberculosis last 2018, his father was enrolled in TB DOTS center. At
first he was cooperating and willing to be cured but as the medication goes through he stop
and never came to the barangay health center. The client’s father became lonely and
hopeless since the death of his wife he became depressed. The client’s father and siblings
are relying only on the money that given by their eldest sister that works as a domestic
helper in Saudi Arabia to sustain their everyday needs. All of his siblings were not schooling
anymore.
C. The Environment
The client is currently living on the lumber yard where he works at Poblacion
Mother Cotabato City. The client sleeps in a small bed in the lumber yard. Inside the lumber
yard he has a light and has no electric fan and any other appliances. He asked his boss to
allow him to stay in the lumber yard because his family is living in barangay Bubong
Kalanganan II Cotabato City. The stall is made of light materials and has no comfort room.
He used to share with the comfort room in the nearby mosque.
V. ASSESSMENT STATEMENT
The client is experiencing on and off fever and a cough which makes him feel weak
and hinder him to work. He is having difficulty in sustaining his daily expenses. He often
skips his meals which makes become weak. He has nobody to ask for help. He doesn’t know
what to do and where to go and ask some help for the solution of his problem.
The client’s father was hard working person he started to stop working after the
death of his wife, he become depressed, weak and thin. Before he was a responsible and
hardworking person, he provides everything for his family. The client’s sibling is no longer
attending school. According to the client they still want to go back to their studies but due to
financial problem they couldn’t go back to studies. They need to work at younger age just to
have some money to buy their own food. They also have relatives who want to take care of
them but they don't last long because they don't help in housework and sometimes reprimand
them. And because of their situation the client’s elder sister decided to work abroad and
become the bread winner of their family. She works as a domestic helper at the age of 20
years old she endure the hardship of working abroad at the early age just to help her family.
The client’s younger siblings can study into the nearest school in their house but they didn’t
study and chose to stay in the house and play all day long.
The problem that the client is facing right now needs to be assisted and facilitated so
that he can avail medical assistance that will help him for the treatment of his illness. There
is a need for immediate treat for the client’s condition because if it is not given an urgent
solution it will lead his condition on becoming worst. And also client’s family needs to
undergo some check-up based on the history that their father was diagnosed of having
tuberculosis. The client’s family will be saved from suffering this kind of diseases. And
because of client’s participation in looking for the solution of his problem, he will become
knowledgeable on how to find a solution if he face some difficulties in the future. The client
will become strong and brave whenever he faces different problems.
Through the student’s assistance the client will be facilitated to avail medical
treatment for his illness. If the client’s will be given a medical assistance his illness will be
treated and he will able go back to work and can sustain his daily needs. And he will gain
knowledge on where to ask some help for the solution of his problem. He will be no longer
needs some help if he experiences this kind of difficulties in the future.
VI. PLAN OF INTERVENTION
Goal: To help the client in avail free medication and food.

VII. EVALUATION
The undersigned facilitate the client in availing medical treatment from the barangay health
center. And assisted the client in application of green card membership that serves a primary
requirement that needed in availing any kind of services from the barangay. The client avail
medical assistance to the barangay through the assist of the social work student.
After facilitating the client for his medical examination the client was undergone of Gene
count test for his sputum. After a couple of days the result came and found out that the client is
having a tuberculosis. The barangay midwife facilitated the enrollment of the client in TB
DOTS center for a six months medication for the treatment of his tuberculosis and he is also
given multivitamins with iron to boost his stamina. Now the client is taking his medicine and his
condition is getting better.
With the help of the intervention given to the client his condition is getting better now and
can able to do petty job such as helping open the lumber and closing in the afternoon. The client
can now avail any kind of services from the barangay and already know the steps and the
responsible persons that he needs to approach whenever he needs a help regarding on his
condition.
VIII. RECOMMENDATION
Various intervention provided to the client to avail medical treatment to improve his
condition. Now the client can avail any services coming from the barangay using his green card.
The client is now having his medical treatment for his illness. There is a plan of intervention
that has not been implemented because of the shortage of time, the student needs to terminate
this case and pass it under the responsibility of the barangay health worker.
