III- PROBLEM PRESSENTED
The client is experiencing an on-and-off fever and a cough. He appears thin, pale,
and weak. He has no one to ask for help in buying his prescribed medicine or food.
Furthermore, he does not have enough budget to pay for his hospital bill.
IV- BACKGROUND INFORMATION
A. The client
The 30-year-old client appears pale and weak. He often feels uncomfortable and
irritable because of his sickness. He feels helpless and hopeless as he has no one to
ask for help. The client is a hardworking individual who works as a helper in a lumber
yard, but his income is not enough to sustain his daily expenses. He is the only one
providing for his needs. He wants to receive treatment for his illness but has no one
to turn to for assistance. Therefore, he decided to seek consultation at Roldan Albutra
General Hospital in hopes of getting well
B. THE FAMILY
The client father has no work and his mother was already dead. His father could
not work because he was weak and also suffering from tuberculosis. His father was detected of
having a tuberculosis last 2025, his father was enrolled in TB DOT’S center. At first he was
cooperating and willing to be cured but as the medication goes through he stop and never visit
to Barangay health center. The client’s father because of loneliness and being hopeless since the
death of his wife he got depressed. The clients father and siblings are relying only on the money
that give by their Eldest sister who works as a domestic helper in Saudi Arabia to sustain their
everyday needs, all of this sibling were not schooling anymore.
C. The Environment
The client is currently living in the lumber yard where he works, located in
Poblacion, Carmen, North Cotabato City. He sleeps on a small bed inside the lumber yard. Inside,
there is only a light; he has no electric fan or other appliances. He asked his boss to allow him to
stay in the lumber yard because his family lives in Barangay Masarapan, Carmen, North Cotabato
City. The stalls are made of light materials and have no comfort room. He uses the comfort room
in the nearby mosque.
V- ASSESSMENT
The clients is experiencing on and off fever and a cough which make him feel weak and
hinder him to work. He is having difficultly his daily expenses he often steps his meals which
makes become weak he has nobody to ask for help for the solution of his problem. The clients
father was hard working person he started to step working after the death of his wife, he become
depressed, weak an thin before he was a responsible and hardworking person he provided
everything for his family. The clients sibling is no longer attending school according to the clients
they still want to go back to their studies but sue to financial problem they couldn’t go back to
studies they also have relatives who want to take care of them but they don’t last long because
they don’t help in housework and sometimes reprimand them and because od their situation the
clients elder sister decided to work abroad and become the bread winners of their family. She
work as a domestic helper at the age of 20 years old she endure the hardship of working abroad
at the early age just to helper her family. The clients younger siblings and study into the nearest
school in their house but they didn’t study and choose to stay in the house and play all day long.
