IV. PROBLEM PRESENTED:

Client Gregorio came to the Social Worker Office asking for medical assistance for
his son’s hospital bill amounting to Php27,834.66. Patient Nathan Gregg was admitted
last September 14, 2023, due to Acute Gastritis. Client expressed difficulty in paying
patient’s hospital bill hence creation this case summary report.

V. HISTORY OF THE PROBLEM:

Being a child, it is hard to avoid different kinds of foods. Healthy or unhealthy foods
are eaten by children, especially the younger ones. The families attended neighbors’
celebration on September 09, 2023, and his son Nathan Gregg was looks fine and very
attentive at that time and he never try to talk about his condition because he never felt it
suddenly an hour after he says that he feels pain coming from his stomach and we
thought it was just a tummy ache.

On September 10, 2023, the patient has experienced sudden inflammation or


swelling in the lining of the stomach 4 days prior to admission, patient had onset of
abdominal pain on and off. On the day of admission persistence of abdominal pain was
noted and onset of vomiting for 2 episodes watery. Thus, prompted admission.

On September 14, 2023. As reflected on the patient's Clinical Summary/Medical


Records, a few hours before admission patient complained of a headache. A few hours
before admission the patient had sudden onset body weakness and upward rolling
eyeballs and thus was brought to the hospital. He was diagnosed with Acute Gastritis
under the medical supervision of her attending physician. Gregorio, the father’s patient
struggles to pay the huge amount of his son’s hospital bill due to a lack of resources.

VI. BACKGROUND:

CLIENT

Client Gregorio, father of the patient Nathan Gregg he is 50 years old and
currently living in Purok 2, Spring, Alabel Sarangani Province. Gregorio was a good
father to his children, and he always taught them to be good children. he taught Nathan
to be good to others, help others as they can, and don't forget always to put God in
everyday life so that he will guide her no matter what problem you face.
Gregorio was in his high school days she couldn't continue his studies because
no one would support him in terms of finances his parents could not afford to pay for his
tuition fee in college, and he always find ways just to earn and provide for his parents at
his young age he always look forward that no matter how bad your or hard your past it
doesn’t matter ho you face it in the present and he always valued simple and small
things in his life and teaching his children to be more thankful of what they have than
finding what they don’t have.

FAMILY

The patient's wife works as a school staff with a monthly salary of seven
thousand (Php7,000), which is insufficient for the family's monthly necessities while the
client, Gregorio works as a barangay worker earning five thousand a month which is not
sufficient to provide for his children and families necessities.

Nathan is the oldest of the two he is a jolly child and always appreciates his
father and mothers did to him specially helping him on his schoolwork’s like coloring the
teachers’ assignments to them and helping him reading some words from numbers to
letters. They are financially unstable.

The relatives of their family also extend their hands helping each other,
especially now that they are in crisis unfortunately, it is not quite enough to settle their
obligation. She is hoping and praying to solve their problem immediately so that they
can pay his child’s remaining bill.

COMMUNITY

The client’s family lives in Purok 2, Spring, Alabel Sarangani Province. Their
homes are so close to one another. They use a water pump as their water source, and
the school, churches, and markets are just walking distance from their home, making it
easy and accessible for the family and their neighbors and most of the family’s source
of income are carpentry, farming, fishing, and tricycle driving. The client's family has a
positive attitude toward their neighbors.
Spring is a barangay in the municipality of Alabel, in the province of Sarangani.
Its population as determined by the 2020 Census was 5,635. This represented 6.38% of
the total population of Alabel.

VII. ASSESSMENT STATEMENT:

The client, Gregorio, is urgently seeking help for the remaining bill of his son
Nathan. He stated they had insufficient resources because her wife cannot provide also
for their needs, and they looked forward for some assistance offered by the government
they can avail. Since then, they are already suffering, even for their basic needs.
Nathan’s father Gregorio also cannot afford support and gave them their necessities.

The client showed great motivation in the planning assessment and kept involved
throughout the interview. Thus, the client's participation is necessary for the intervention
and can be seen in the helping process.

Unfortunately, they are unstable financially on paying the hospitalization.


Gregorio is eager to find other assistance to process the needed papers, but they need
more resources to provide a large amount of hospital bills. She is hoping to resolve their
problem immediately. If the problem cannot be solved immediately it will cause trauma
and he will be more prone to other sicknesses and viruses at his age.

and the patient will be discharged from the hospital.

X. EVALUATION:
The client Gregorio was referred by Soccsargen Hospital to DSWD-CIU
Department of Social Welfare and Development- Crisis Intervention Unit Office last
September 21, 2023, to avail assistance for the hospital bill and was given aid
amounting to Php10,000.00.

XI. RECOMMENDATION:

Anent to the collective information and through assessment of the case, the
undersigned worker humbly recommends medical/Financial assistance through DSWD
Region XII.

The Social Work Intern Humbly recommended to avoid sugary snacks and drinks
to their child and encourage healthy eating habits.

XII. TERMINATION:

In the case of Nathan Gregg, the patient, the client received all the Assistance he
needed for his son with the help of Social Welfare agencies. The objectives and goals
set by the client through the guidance of the social work intern during the initial phase of
the helping process have been met. The case is terminated.
