AFFIDAVIT OF LOSS

I, WILFREDO PASAMONTE BONDOC, of legal age, Filipino and with


residence and postal address at 103 Shaw St., Sta. Rosa Estates, Santa Rosa,
Laguna, after having been duly sworn to in accordance with law do hereby depose
and state:

1. I am a graduate of Polytechnic University of the Philippines on


1991 with a degree of Bachelor of Science in Mechanical
Engineering wherein I was issued a Diploma;

2. That I placed my Diploma in a brown envelope together with my


other personal documents, and as far as I can recall I kept the
said envelope in my desk cabinet in my home;

3. However, when I looked for the said envelope in the place where
I kept it, I can no longer find the same and I believe I may have
lost it;

4. That I tried all efforts to find said envelope but all proved to be
futile;

5. That I am now requesting for a duplicate of my School Diploma


from the Polytechnic University of the Philippines in lieu of the
one lost;

6. That I am executing this affidavit to attest to the truth and


veracity of the foregoing statements and for whatever legal
purpose this may serve.

AFFIANT FURTHER SAYS NAUGHT.

SUBSCRIBED AND SWORN TO before me this 12 th day of February


2019, in the City of Manila, Philippines, affiant exhibited to me his Driver’s
License with ID No. N04-93-266630 as competent proof of her identity.
