Sub: Letter in reply to show cause notice issued for the leave without permission

Respected Sir,

Hope this letter finds you in best of your spirits. On 29 th October 2021, I have been
issued a show cause notice from the HR office for the leave without permission. I want
to clarify my situation through this letter. As I was feeling pain in my chest, and was
not feeling well so, I left the office without informing any official. I apologize for not
dealing the situation well and request you to kindly not to take any action against me. I
assure you that I will avoid such inattention in future.

Please find enclosed herewith my fitness certificate for your perusal.
