WAIVER FORM

I, ________________________, of legal age, Filipino and a resident of


____________________, understand that I am, or will be responsible for all my tax remittances to
Bureau of Internal Revenue (BIR) and payment of government fees associated with the Independent
Contractor Agreement entered between me and Cebu Machine Intelligence Laboratories Inc. (company)
as an Online Chat Agent. As an independent service provider the following will be my sole
responsibility:

1. Registration of the service offered/business in BIR;


2. Issuance of official receipt for the compensation received;
3. Filing of quarterly and yearly income tax; and
4. Comply with the government rules as a service provider

The company is not responsible for any delinquencies in tax remittances and or any payment
due to the government incurred by the independent contractor in relation to the services provided.

By signing this I hereby agreed as follows:

1. I have read this waiver and fully understands it;


2. I make and execute this waiver voluntarily, without any form of duress or coercion;
3. I recognize that upon signing this, it shall serve as a bar to any claim, litigation in any
tribunal over this matter.
