Dear Ma’am:

In line with the increased number of the learners in the Grade 10-DOVE who failed to
claim and submit their modules and activity sheets on the past weeks, we would like to ask
permission from your good office to allow us to conduct home visitation in our select learners on
January 31, 2022 (Monday) at 4:30 o’clock in the afternoon. We believed that this action taken
by us will help us understand the reasons of the parents for the failure of the submission and
claiming of activity sheets, and hence, encourage our learners to answer their activity sheets
properly.
