AFFIDAVIT OF MUTILATED PLATE

I, _________, of legal age, Filipino, and a resident of _______, after being


sworn in accordance with law, hereby depose and state that –

1. I am the owner of one Nissan Navarra motor vehicle with plate


number _________;

2. On 11 July 2020 while driving said motor vehicle, another vehicle


suddenly hit my car;

3. Due to the collision, the plate number of my vehicle was mutilated


and damaged;

4. It is in this regard that I execute this affidavit to inform the Land


Transportation Office of the fact of damage and mutilation caused to my
vehicle’s plate number;

5. I further execute this affidavit in compliance with and/or for


purposes of satisfying the requirements for the issuance of a new plate;

6. Finally, I am executing this affidavit to attest the truth and


veracity of the foregoing statements and for all legal intents and purposes it
may serve;

7. Affiant sayeth naught.
