WAIVER OF APPEARANCE

I, _______, of legal age, American citizen, widow, and a resident of _______,


after being sworn in accordance with law, hereby depose and state that –

1. I am a Senior Citizen, having been born on August 19, 1940;

2. Due to quarantine restrictions brought about by the COVID-19


pandemic, all senior citizens are not allowed to leave their homes or go outside
of their houses;

3. I intend to apply for the re-acquisition of my Philippine Citizenship;

4. Due to my current age, I am not allowed to go outside of my home


and personally transact with the Bureau of Immigration, Philippine Embassy
and/or other government agency or office;

5. Considering the foregoing circumstances, I am waiving my


appearance before the Bureau of Immigration for the taking of my Oath of
Allegiance;

6. Finally, I am executing this affidavit to attest the truth and veracity


of the foregoing statements and for all legal intents and purposes it may serve.

7. Affiant sayeth naught.

IN WITNESS WHEREOF, I have hereunto affixed my signature this
