AFFIDAVIT OF NO RENOVATION
I, GIIL F. SAMIA, of legal age, married to NELIA B. SAMIA, Filipino, with address at 777 E. Pantaleon St.,
Barangay Barangka Drive, Mandaluyong City, after having sworn to, in accordance with law, do hereby
depose and state, THAT:
1.I am the OWNER of the establishment;
2. SIBS CAFÉ & ROASTERY OPC is our tenant and leased 306- A2 GFS Building thereof,
evidence of which is the CONTRACT OF LEASE hereto attached and made integral for reference;
3. The said LESSEE has not introduced any improvement and/or renovation of whatever
kind and nature on the said premises and occupy the same in “AS IS WHERE IS “ basis and;
4. I am executing this affidavit upon the request of the said LESSEE for securing the
required BUSINESS PERMIT and to attest to the truth and veracity of the foregoing statements of facts
and for whatever legal intent and purposes it may serve.
