IDENTITY THEFT AFFIDAVIT

I, India Settles, hereby affirm, under penalty of perjury, that I am an identity theft victim in
regards to cause number 49K07-1008-SC-04423, as I never committed to an agreement with
Arrowhead Apartments or any affiliation thereof and my credentials were fraudulently used
without my consent. At no time have I ever personally resided within Arrowhead Apartments.
During the alleged timeframe, I resided at 13720 N. Dover Hill Dr. Camby, In 46113 with my
Grandmother, Wanda Green.
