I am contacting you about the compromising of my social security number. I am a victim of


Identity Theft. I contacted the Federal Trade Commission and filed a complaint # and contacted
the police department and obtained a police report # which both are attached. Please block and
remove all information from my credit report, and send me an updated copy of my credit report.
The following items do not belong to me and is a result of fraud that I did not authorize:

Please block and remove all information resulting from Identity Theft pursuant to FCRA 605B
(15 U.S.C. & 1681 c-2) which states that these accounts must be removed within 4 Business
Days of receipt.

Also, please send an updated copy of my credit report to the above address. According to the
act, there shall be no charge for this updated report. I also request that you please send notices
of corrections to anyone who received my credit report in the past six months.
