with the Federal Trade Commission (FTC) and, if required, with local law enforcement.
Under Section 605B of the Fair Credit Reporting Act (FCRA), 15 U.S.C. § 1681c-2, you are
legally required to remove and block all fraudulent accounts, inquiries, and related
information within 4 business days of receiving this request and documentation.
THIS DEMAND COVERS:
• All fraudulent accounts listed on my credit report (see attached).
• All unauthorized hard inquiries, made without my knowledge or consent, in
connection with identity theft.
These entries were made without my authorization and are causing serious harm to my
credit and financial reputation. Under federal law, these items must be permanently
blocked from my file and may not be reinserted unless you verify their accuracy and
legitimacy with documented proof, which you do not have because I never authorized
these transactions.
REQUIRED ACTIONS:
1. Delete and permanently block all fraudulent accounts and hard inquiries
connected to identity theft.
2. Stop and prevent any future reporting of these items.
3. Send me written confirmation that all disputed accounts and inquiries have been
removed and blocked.
4. Reissue a corrected copy of my full credit report, free of charge, reflecting these
removals.
LEGAL CONSEQUENCES FOR NONCOMPLIANCE:
If you fail to comply within the timeframe allowed by law, I will file immediate complaints
with:
• The Consumer Financial Protection Bureau (CFPB)
• The Federal Trade Commission (FTC)
• My state attorney general
• I will pursue civil legal action to seek damages, attorney fees, and punitive
penalties.
