do hereby NAME, CONSTITUTE AND APPOINT, MAUREEN JOY A. MANIAGO,of legal ages, Filipino
and resident of San Isidro, Minalin, Pampanga, to be my true and lawful attorney-in-fact, for me and in my
name, place and stead, and for my own use and benefit, to do and perform all or any of the following acts
and things, namely:
1. To represent me before the Municipal Office of Lubao, Pampanga, Bureau of Fire Protection
Office (BFP) and/or any other government office or agency/ies related for the processing of
my building permit, electrical permit, sanitary and plumbing permits, and all other permits or
requirements directly or indirectly associated with the construction and operation of my
house situated at San Roque Dau, Lubao, Pampanga;
2. To get, sign or receive from the above mention Government Agency/ies any copy of the
documents related the above-mentioned transaction;
3. To generally do and perform all and every acts which may be requisite, necessary or
proper to carry out the above purposes.
HEREBY GIVING AND GRANTING unto my said attorney-in-fact full power and authority
whatsoever requisite and necessary proper to be done in and about the premises as fully to all intents
and purposes as I might or could lawfully do if personally present and shall lawfully do or cause to be
done under and by virtue of these presents.
