AFFIDAVIT OF VOLUNTARY SURRENDER
I, JOHN REIAH MANANSALA GONZALES, of legal age, single, Filipino,
and resident of San Isidro, Minalin, Pampanga, after having been sworn in
accordance with the law, do hereby depose and say:
1. That I am the accused in Violation of Provincial Ordinance No. 756 in
relation to R.A 11332 with Criminal Case No. 21-117, now pending
before the 3rd Municipal Circuit Trial Court of Sto. Tomas and Minalin,
Pampanga;
2. That the bail recommended for the provisional liberty is in the amount
of Three Thousand Pesos (P 3,000.00) as fixed by Honorable Court;
3. That I voluntarily submit myself on the jurisdiction of the Honorable
Court by filing the cash bond in the amount of Three Thousand Pesos
(P 3,000.00) thus for all legal intents and purposes, I hereby manifest
my voluntary surrender hereof;
4. That I am executing this Affidavit for the information of the proper
authorities, and for whatever legal purpose it may serve.
