AFFIDAVIT
I, HUCKYARD SANZ PAGAS AMANTILLO, of legal age, single,
Filipino, and a resident of Sta. Maria, Tagbina, Surigao Del Sur,
after having been duly sworn to in accordance with law hereby
depose and state that:

1. That, I am presently applying for the Armed Forces of


the Philippines;

2. That, in support of my application, a Mayor’s Clearance


is needed to be submitted as one of the documentary
requirements;

3. That, however, the Office of the City Mayor of


Cabanatuan informed me that I need to execute an
Affidavit stating that I am not a registered voter of
Cabanatuan City, Nueva Ecija;

4. That, truth and in fact, I am a registered voter in


Surigao Del Sur and recently exercised my right to vote
last May 2022 National Election;

5. That I am executing this Affidavit to attest to the truth


of all the foregoing facts for all legal intents and in
compliance with the requirements for my application for
the AFP;

IN WITNESS WHEREOF, I have hereunto set my hand this 30 th


day of March 2023 in Cabanatuan City, Nueva Ecija.
