SWORN AFFIDAVIT OF DISCREPANCY


I, MARY JHUSTYN MINGUILLO CARGADO, of legal age, single, Filipino citizen
and with address located at 8 Don Pablo Subd., Barangay Canlalay, City of Binan, Laguna,
after having been duly sworn to in accordance with law, hereby depose and state:

1. That I was born on September 8, 1997, at San Pedro, Laguna, with parents
AGUSTIN R. CARGADO JR. and KHAREEN C. MINGUILLO;

2. When I secured a copy of my Certificate of Live Birth from the Philippine


Statistics Authority, and after scrutinizing the same, I discovered that my
recorded name is MARY JHUSTYN MINGUILLO;

3. That during my childhood and when I went to school, I used the name MARY
JHUSTYN MINGUILLO CARGADO as it was the name that my parents given
me;

4. That since childhood, I have been accustomed to using MARY JHUSTYN


MINGUILLO CARGADO and used the said name when I signed documents
and secured government identification card;

5. That the name MARY JHUSTYN MINGUILLO CARGADO and MARY


JHUSTYN MINGUILLO refer to one and the same person;

6. That I execute this affidavit to attest to the truth and veracity of the foregoing
facts for whatever legal purposes it may serve it best.

IN WITNESS WHEREOF, I have hereunto affixed my signature herein below this 8th
day of January 2024, at the City of Binan, Laguna.
