AFFIDAVIT

I, ELMER VILAR SITOY, of legal age, Filipino citizen, with residence


address at Sitio Darisnai Compaound Dahlia Ave., West Fairview, Quezon City,
Quezon City, after having been duly sworn to in accordance with law, hereby
depose and state;

1. That I applied at MERALCO for Electric Connection on October 02, 2023


with an Official Receipt No. 0000275283;

2. However, I misspelled the address on the application form and I wrote it


0025 Darisma, CPD Dahlia Ave, West Fairview Park Subd. Fairview, Quezon
City Greater Fairview, Metro Manila, instead of Sitio Darisnai Compound
Dahlia Ave., West Fairview, Quezon City;

3. That I am executing this Affidavit to attest to the truth of the foregoing


facts based on my personal knowledge and to request to proper
authorities to change my address on my application form at MERALCO
and for whatever legal purpose it may serve.
