AFFIDAVIT OF LOSS

I, GINALYN S. BALTAZAR, 48 years old, married, Filipino, and a resident of


Brgy. Darasa, Tanauan City Batangas, after having been sworn to in accordance with
law, hereby depose and say, THAT:

1. I am a bonafide member of 4Ps Program (“Pantawid Pamilyang Pilipino


Program” ) project by the Government through the Department of Social Welfare and
Development (DSWD) of Tanauan City, Batangas;

2. Consequently, I was issued an Identification Card (ID) and an Automated


Teller Machine (ATM) of Landbank by the DSWD as proof of my qualification for the
said project and likewise, to where I obtain benefits as qualified 4Ps member;

3. Sometimes in July 2019 when I was about to withdraw from the said ATM, I
looked it inside my wallet where I usually placed it for safe keeping and convenience,
however, the said ATM was no longer there;

4. Hence, I looked around my house in search of my ATM card I mentioned in


the hopes that I might only misplaced it and even asked family members if they happen
to chanced the said ATM card but to no avail;

5. The aforesaid ATM card is now presumed lost and beyond recovery;

6. I am executing this affidavit to attest to the truthfulness and veracity of the


foregoing statements, for the purpose of requesting for the issuance of a replacement
of Landbank ATM card for 4Ps with DSWD, and for whatever legal intents and purposes
it may serve.

Affiant further sayeth naught.

IN WITNESS WHEREOF, I have hereunto set my hand this 1st day of


October 2019 at Tanauan City, Batangas, Philippines.
