AFFIDAVIT OF LOSS

I, MA. FLORDELUNA DELA CRUZ SERAFIN, of legal age, Filipino, with


residence address at Brgy. Tactac, Balayan, Batangas, after having been duly
sworn in accordance with law do hereby depose and state:

1. That I was issued a Landbank ATM Card for the benefits I receive
under the 4Ps Program;

2. That I placed said Landbank ATM Card inside my wallet;

3. That on January 2, 2025, I lost my wallet in Magabe, Balayan, Batangas;

4. That despite diligent efforts to recover my wallet containing my


Landbank ATM Card, I could not retrieve the same such that I now have
reason to believe that it is lost and could not be recovered;

5. As such, I am executing this Affidavit of Loss to attest to the truth of the


foregoing and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 24 th day of


January 2025 at Balayan, Batangas.
