AFFIDAVIT OF COHABITATION

I, ALBERTO CRUZ DOROTEO of legal age, Filipino, single, with


residential address at Brgy. Palikpikan, Balayan, Batangas,
Philippines, after having been duly sworn in accordance with law,
state that:

1. I am the common law husband of the late FRANILYN LOPEZ


SORIANO and we lived together as husband and wife without
the benefit of marriage since 2018;

2. FRANILYN LOPEZ SORIANO died on January 5, 2025 due to


Acute Cardiac Failure;

3. During our cohabitation, there was no legal impediment to


marry each other;

4. I am executing this affidavit to attest to the truth of the


foregoing facts and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 7 th


day of January 2025 at Balayan, Batangas.
