AFFIDAVIT OF COHABITATION

We, ________________ and ______________, both Filipinos, of legal ages,


and residents of ________________________, after having been duly sworn to in
accordance with law, depose and say THAT:

1. We have been living together as husband and wife since sometime


_______, although our union has not been formalized by marriage;

2. Out of the said union born _________________, born on


________________, now ____ (____) years old, ______________, born
on ____________, now _____ ( ) years old, and _______________, born
on ____________, now ____ ( ) years old;

3. During the said union, we were both single and of legal age, so we had no
impediment to getting married. Despite planning to legalize our union, we
kept postponing it and remain unmarried. However, we have always
considered ourselves husband and wife and presented ourselves as such
to others;

4. But this time, with the present realities, we have decided to finally be
united in matrimony according to our laws. And in furtherance thereof, we
invoke Art. 34 of the Family Code which legal provision pertaining to this
matter, exempts us and those similarly situated in securing a marriage
license, which in lieu thereof, is this Affidavit;

5. We are executing this affidavit to attest to the truth of the foregoing facts
and to state further under oath, that we have been living together for more
than five (5) years already as husband and wife counted from the year
_______and thus, we are invoking Art. 34 of the Family Code of which we
can be united in matrimony using this Affidavit in lieu of a marriage
license.
