1. That we have been living in the concept of husband and wife but without the
benefit of marriage ever since 2013 or a period of more than five (5) years;

2. That we intend to lawfully enter into a contract of marriage pursuant to Art. 34 of


the Family Code and E.O. No. 209;

3. That ever since we lived together and up to this date, we remain both of single
status and hence there is no legal impediment for us to marry each other;

4. That we are executing this affidavit to attest to the foregoing facts and for whatever
legal purpose it may serve best.
